How to apply for European Health Insurance Card (EU card)?

I come from another EU country and work in Sweden now. How do I apply for the European Health Insurance Card?

    To receive the European Health Insurance Card (EU card) you can apply for the card through our website. You can find the link here.

    You can also order the EU card through our self-service by phone. More information can be found here.

    If you recieve a message that you cannot proceed with your order, you might need to fill in the registration form FK5456 (Uppgifter vid utredning av försäkringstillhörighet) and send it to us with the relevant attachments. Attachments are listed on the form in section 10. On the application form you have to state that you wish to receive EU card. As soon as you registration is approved, we are going to send the EU card to your home address. You can download form here.

  • Hi, I applied for the EU card using the 5456 form and I got a decision that I am not eligible for this since I am from outside EU.
    Just a brief about me : I am staying and working in Sweden since the last 14 months and I have a co-ordination number for the KU10 that Skatteverket gave me.
    How do I proceed now in this case??
    I had a call with your customer care and all that they had to say is that the Case worker decided on all this.
    So, is the EU card only for EU residents ? If yes, then how do non-EU people get the EU card and hence the Medical Insurance?

    You are eligible for EU card when you belong to the Swedish Social Insurance system and are registered at Försäkringskassan. That is why the EU card is not only for EU residents.

    If you come from the country outside of EU, we need normally residence permit that you send to us together with the form 5456.

    On the public forum, we can only answer on general questions. Unfortunately, we are unable to answer what happened in your specific case and why your application has been rejected.

    I am working in Denmark, but living in Sweden.I have a personal number in Sweden. Do I need to apply for a registration in  Försäkringskassan, because I want to have a health insurance here. And does that insurance is covering my wife and children while we are waiting for a residence permition from Migrattionsverket for them ( they are not eu-citezen but I am)

    You are normally socially insured in the in the country that you work, in this case Denmark. However, if you start working in Denmark, you must report this to Försäkringskassan before you begin working there by sending form 5459.

    Moreover, you should contact the authority in the country where you work to find out what applies in your situation and the social insurance coverage of your family.

    Regarding health care, the general rule is that if you work in another Nordic country (including Denmark) but live in Sweden, you are entitled to the medical care, dental care and medications covered by the public healthcare system in Sweden.

    If you need care in Sweden, you must present your “Certificate on the right to care benefits in Sweden”. You apply for the certificate on My pages (Mina sidor). If you are unable to use My pages (Mina sidor), you can apply using the form 5435. Attach your employment contract and the last salary specifications with this form.

    I applied for a Swedish EU health card after having obtained my personal number because the one of the country I previously lived (Switzerland) expired at the end of last year. I sent all the documentation required (form 5459) via ordinary post almost 2 weeks ago and haven't heard anything yet. Would be possible to have some updates?

    The link you mentioned above is still not working (if you click on "beställ EU-kort". Many people on the internet compain about that, so it seems to be down for quite a while =)

    If you are registered at Försäkringskassan and socially covered in Sweden there many ways of ordering the EU-Card (European Health Insurance Card).
    you can order it in this link here. You have to click on the blue box where it is written “ beställ EU-kort”.
    You can also order the EU -card by sending a text with EUKORT and your personal identity number to phone number: 71020
    Example: EUKORT your_personal identity number additional_personal identity numbers

    If you are not registered at Försäkringskassan, you have to apply for the EU-card by sending us form 5456 with relevant attachments. You also need to write in section 10 in the form that you wish to apply for the EU-card.
    You can fill out the form online in My Pages or you can use regular form which can be found the form here.

    If you have lost your EU-card, you can order a new one here. Then, the old one will cancelled and it will no longer be valid.
    If you need us to cancel the card and will order a new one for you, please contact our customer center at the telephone number 0771 524524 and press 8 to get connected to the English line

    I ordered my EU-card 3 months ago. I got a letter in my mail box saying the work load was very high and that my case was delayed because of this...
    I am wondering now, how long will it take to get the card? I do not even know if the documents I provided are going to be accepted because my case is still on queue. When I called the customer service, they just told me my case was going to be prioritized and that my documents seem to be correct. However, as the case has not been assigned to anyone yet, it could be that I need to provide more documentation in the future. I still have no news at all.

    How long does it usually take to get the card?
